Work Injuries
Dr. Guthrie is a WCB provider.
If you have injured yourself at work, Dr. Guthrie can assess and provide covered treatment through workers compensation.
What to expect:
Even if you are a current office patient, expect the first visit to take about an hour. You will have additional forms to fill out. Dr. Guthrie will take a history of the injury and perform an exam. Treatment plan and consent will be discussed similarly to an initial chiropractic assessment. Dr. Guthrie will submit the required forms to WCB on your behalf, for in office treatment.
If you have booked for a WCB assessment, please expect to arrive early to fill out multiple forms.
What to Bring:

Please bring your WCB claim number and claims advisor number (if you have this) as well as your work address and contact number.
Out of pocket costs:
If WCB approves your claim, Dr. Guthrie will direct bill WCB for any chiropractic services utilized.​

If coverage is denied for any reason, you will be billed for any outstanding invoices. *
